Osteoporosis and Frailty: What 2026 Research Reveals About Supplements

VITAMIN D AND FRAILTY: WHY STEADY BEATS MEGA-DOSES
Researchers Viola and colleagues explored how vitamin D functions in many parts of the body, supporting muscle strength, bone renewal, immune health, and energy production. (1) Low vitamin D levels are connected to muscle loss, weaker bones, and the onset of frailty. Still, supplements seem to offer the most benefit when someone is genuinely deficient or especially vulnerable, and large, occasional "mega-doses" have been linked with more falls and fractures. Bottom line: know your numbers, and choose steady, recommended doses over occasional large ones.
CALCIUM AND VITAMIN D: A FOUNDATION FOR OSTEOPOROSIS CARE
A review by Pascual-Pastor and Barceló-Bru called daily calcium and vitamin D an essential foundation of osteoporosis care. (2) The authors summarized the science on how the two nutrients work as a team to lower fracture risk and maintain bone density, and noted that vitamin D deficiency is still widespread even among people who already have osteoporosis. They also highlighted the importance of supplementation for those using osteoporosis medications.
DO PROBIOTICS HELP BONE DENSITY?
Probiotics were a different story. In a carefully designed trial, Resciniti and colleagues gave 114 women in the first years after menopause either a daily probiotic or a placebo for 12 months. (3) The probiotic did not slow bone loss, a helpful reminder that promising ideas still need scientific support, and that no single supplement replaces a well-rounded plan.
